Sprouts Enrollment
Sprouts is a program for children aged 18 months to 2.5 years of age and their caregivers.

To enroll your child, please fill out the form below and use PayPal to submit the $120 fee. If enrolling multiple children, you will need to complete forms and payment for each individual child.
